Lieutenant Andrews was killed in an automobile accident on State Route 13 in Mt. Vernon, Ohio, when his vehicle left the roadway and struck a bridge abutment.

Lieutenant Andrews had served with the agency for 12 years and was stationed at Columbus at the time. He was survived by his wife, son, two daughters, parents, and brother.
