Officer Chadwell was shot and killed as he and three other officers attempted to serve a warrant on a suspect. Officer Chadwell and another officer entered the front of the home as the other two officers entered the rear of the home.

The suspect was located hiding in a shower in the bathroom. As Officer Chadwell entered the room the suspect opened fire, striking him in the face. The other officers immediately returned fire, killing the suspect.

Officer Chadwell had served with the agency for 12 years. He was survived by his wife and three children.
