Officer Slack was stabbed to death after responding to a domestic disturbance call at his neighbor's house while off duty. The neighbor stabbed him twice with a large knife when Officer Slack attempted to arrest him. Officer Slack succumbed to his wounds two days later.

Officer Slack had been with the agency for 11 years. He was survived by his wife and five children. He was buried in Congressional Cemetery.
